Pendahuluan penghantaran
obat secara topikal lebih dominan dibandingkan sistemik obat yang diberikan melalui mata ditujukan untuk mengatasi infeksi prekornea (konjungtiva) atau pengobatan intraokular (glaukoma) Masalah penghantaran obat secara topikal : Mekanisme penghilangan bahan asing yang masuk ke dalam mata barrier prekornea (reflek berkedip, air mata, permeabilitas kornea yang rendah)

Lapisan terluar Merupakan jaringan fibrous yang berwarna putih (posteor) dan transparan (anterior) yang berfungsi untuk melindungi lapisan mata bagian dalam. Penghubung kornea dan sklera disebut dengan corneal scleral junction atau limbus. Lapisan bagian dalam Bagian dalam mata tersusun dari choroid, the ciliary body dan iris. Lapisan terdalam The inner coat of the eye is called retina. It contains two types of photoreceptor cells (i.e., light-sensitive receptors) called rods and cones. They transfer impulses from the retina to the brain via the optic nerve. The rods function best during dim light. A loss of the rods renders a person almost blind at night. The cones function best in daylight and are also responsible for color vision. A partial loss of the cones can result in a loss of visual acuity and color blindness.

The topical application of opthalmically active drugs to the eye is the most prescribed route of administration for the treatment of various ocular disorders.

It is generally agreed that the intraocular bioavailability of topically applied drugs is extremely poor.

For example, in the treatment of glaucoma with pilocarpine eye drops only a very small fraction (i.e., 23%) of the instilled pilocarpine doses is actually absorbed into the aqueous humor, the target tissue ,for the reduction of intraocular tension in glaucoma patients.

AQUEOUS SUSPENSIONS •

Aqueous suspensions contain a sparingly soluble drug as finely divided particles dispersed in a saturated solution of the drug with a suspending agent dissolved in the medium. The advantage of suspensions over solutions is that the suspended particles are better. Retention increases with an increase in the particle size as does the irritation of the eye. The ophthalmic suspensions available are hydrocortisone acetate suspension, Prednisolone acetate suspension.
Ointments: Ophthalmic ointments represent a common topical dosage form. •
Their obvious disadvantage include interference with vision and aesthetic consideration.

Ointments offers the clinical advantage of prolonged medication from each instillation.

Thus, with drugs whose maximum efficacy depends upon constant presence, such as antiviral agents, night time ointments use combined with a day time eye drop use in a common regimen.
