2.1 Lower Urinary Tract Symptoms
2.1.1
Definisi
Lower Urinary Tract Symptoms (LUTS) Symptoms (LUTS) merupakan kompleks gejala yang terjadi akibat 3
penekanan atau obstruksi pada urethra.
2.1.2
Komponen 3
Komponen LUTS dibagi menjadi dua, yaitu komponen obstruktif dan iritatif. Obstruktif, dibagi lagi menjadi:
Mekanikal. Terjadi akibat adanya intrusion ke urethral lumen atau bladder neck.
Dinamik. Terjadi karena adanya stimulasi autonomic ke prostate stroma yang kaya akan adrenergic nerve supply yang supply yang akan menghasilkan tonus ke prostatic urethra.
Iritatif. Terjadi karena adanya respons sekunder dari kandung kemih karena peningkatan outlet resistance resistance sehingga terjadi hipertrofi dan hyperplasia detrusor muscle yang mengakibatkan 3
terjadi mucosal herniation antara detrusor muscle bundle sehingga membentuk diverticula.

Gejala
Gejala-gejala LUTS dibagi menjadi: 1. Gejala iritatif a. Urinary frequency, frequency, yaitu adanya peningkatan frekuensi micturition lebih banyak dari biasanya. b. Nocturia, Nocturia, yaitu bangun di malam hari untuk buang air kecil. c. Dysuria, Dysuria, yaitu adanya perasaan sakit ketika buang air kecil.

d. Incontinence, yaitu ketidakmampuan untuk menahan buang air kecil. 2. Gejala obtruktif
a. Decreased force of urination, yaitu berkurangnya kekuatan pancaran urine ketika buang air kecil. b. Urinary hesitancy, yaitu ketidakmampuan untuk memulai buang air kecil. c. Intermittency, yaitu buang air kecil dalam volume yang sedikit-sedikit. d. Terminal dribbling, yaitu adanya tetesan ketika akan mengakhiri buang air kecil. 3
e. Straining, yaitu mengejan ketika akan buang air kecil agar urine bisa keluar.
