Loadline and deckline to be entered as per Loadline cert to be entered Details regarding the administering of Morphine or Pentazocine to be entered: details of person to whom administered, reason, date and quantity and details of person administering drug To enter loss or theft of these drugs Supintendent or official inspecting water or provisions after complaint to make an entry Inspections of hospitals with a another crew member every seven days to be entered, include date/time, people inspecting and any defects found (passenger ships) Inspection of accommodation with another crew member to be made every seven days to be entered, include date/time, people inspecting and defects found Inspections of radio equip to be made if a radio log not on board Entries regarding musters and drills to be made and/or failure to hold a drill Entries regarding closing and opening of watertight devices to be made. Entry if ship has done damage to pipeline, cable, lighthouse, beacon, buoy or other ship, wharf or jetty Entry regarding stowaway discovered on board: name, age, birthplace, place he came on board and put ashore, proceedings taken against him, result of those proceedings. (Stowaway subject to same provisions as crew as per conduct) When master changes he is to hand over all documents relating to nav of ship. Entry to be made that successor has taken over the ship. Entry regarding seaman’s character to be entered into pt II and to be handed to superintendent when seaman discharged. Only entries allowed are very good, good or bad. Entry other than very good may be made in pt XI. Master has responsibility for maintaining discipline on board and may impose sanctions on seaman after inquiry. Master to inform crew of alleged breach Crew member to have chance to admit or refute allegation and to call witnesses. May make statement in answer to allegation and may be attended by another member of the crew Master to conduct investigation and if breach committed to inform crew member of result and sanction Record of breach and all facts to be made in OLB. Withdrawal noted in OLB and copy handed to crew member and super. Promotion to be noted in OLB: date of promotion, new rank/rating, increase of wages is appropriate, whether promotion temp or perm. Master to make entry regarding the misconduct of an officer Seaman found guilty of desertion to be entered in OLB Seaman guilty of desertion is: he is absent from his ship with intention not to return, he is absent for a period exceeding 48hrs without leave, lawful cause or reasonable excuse. Mus prove he belonged to the ship and that he was unlawfully absent Seaman left ashore – master to enter into OLB wages owing the seaman and particulars left in his custody. To record why wages not paid and gear not delivered. Where dead persons gear taken into his charge, Master to list the gear and money and have entry witnessed by third member of the crew. Births, deaths, disappearances or injury to be noted in appropriate section and details placed in narrative section.
