Unsafe Act Most of the workers & staff are unaware of proper use of safety helmet. Many workers are working at height without safety belt. (many of them are wearing on body but not anchoring to anchoring point) Some of the workers are working without safety shoe. Workers are doing grinding work without face shield. (They are using safety goggles). Unsafe Condition No proper access is provided to many of the elevated working platforms.

No electrical cable should lay on ground and wet surfaces.
All cylinders should always placed vertically Proper return cable should be use Standard equipments should be use. Every vehicle should be thoroughly examined before deployment. All staircases should have proper handrail, mid-rail and toe guard. If scaffolding is more than 6M of height then every 6M cross bracing is required. Good condition and tested slings should be use. All pressure gauges should be in working condition to avoid back fire. Standard man buckets should be use for height work.
Proper lifeline with fall arrestor should be provided for vertical as well as horizontal movement on elevated structures.
