EndoTool™ User Manual

EndoTool™ is to be ordered, used, and monitored by licensed medical care providers consistent with Federal, State, Local, and Hospital Applicable Rules, Policies, and Regulations. EndoTool™ is designed to calculate insulin doses for patients with: * Elevated blood glucose levels who are critically ill. * Relatively regular nutritional intake; IVs with glucose, TPN, Tube Feeding, etc. EndoTool™ is NOT designed for use with patients who are receiving meals, intermittent feedings, or occasional oral snacks. When intermittent feedings or meals are resumed, the use of EndoTool™ should be replaced by an alternative diabetes management. Online help, as well as an online User's Guide, are available via the application's Help menu. Introduction EndoTool™ is a method and device for controlling the level of glucose in a patient in intensive, critical or urgent care units of a hospital. More particularly, EndoTool™ relates to a method and device for controlling the level of glucose in a patient using a software algorithm that is a feedback controller for blood sugar control for stressed and critically ill patients. Currently in the United States, 6,400 hospitals with an estimated 50 beds are dedicated to patients in intensive care. Each bed is utilized for approximately 100 patients per year. Thus, there are approximately 32 million patients treated in ICU's each year. Glucose control of these critically ill patients often determines their outcomes or survival and has a very strong role in the mechanisms that protect against hospital acquired infections. High blood sugar (glucose) in a patient whose body is in stress leaves the patient vulnerable to hospitalacquired infections. This possibility is very high to vulnerable patients in intensive, critical, and urgent care units. In the United States, approximately 500,000 patients undergo coronary artery bypass grafting each year, 20% of which have diabetes mellitus. Studies suggest that perioperative hyperglycemia in the patients with diabetes is associated with higher infection rates. Patients with diabetes undergoing cardiac surgery with postoperative glucose greater than 200 mg/dL have a 17% to 86% increased risk of infection. The glucose acts as a nutrient for viruses and bacteria promoting propagation and at the same time inhibits the immune system. Infections in this setting increase; death, recovery time, and time and costs in intensive care. EndoTool™ was created to assist healthcare personnel in the control of diabetes of patients in acute and urgent care settings. EndoTool™ is an electronic tool used to calculate a recommended Insulin dosage based on glucose levels gathered by nurses. In addition, EndoTool™ has "safety factors" and "accelerator factors" built in that 1) prevent too much insulin or 2) increase insulin dosage because patient history shows he/she is resistant to it, therefore needs more. The use of EndoTool™ has proven to be beneficial to nurses and doctors with respect to the reduction of hospital acquired infections. Aside from the obvious patient benefits EndoTool™ includes; high security, easy and effective data management, ease of software upgrades, data backup capabilities, and technical support. Each user has an individual user ID and password in order to access portions of the application. The management of the patient data is easier through the use of electronic tools, i.e. automatic backups, purging of old data etc. Software upgrades are accessible through the MDSci website and the technical support is made easier by the use of Microsoft .Net technologies. EndoTool™ is designed to be HIPAA compliant. EndoTool™ uses algorithms and basic functionality packaged in a user friendly, stand-alone program using Microsoft .Net technologies. The application can be run in a Windows 2000 registry platform. The application was developed for use on dedicated Tablets, PCs or network servers. EndoTool™ comes in two editions; Basic and Plus. The Plus edition includes all the features of Basic, as well as the ability to send data to your HIS via HL7. Both editions also allow for an optional Central Database feature that enables storing data centrally in a SQL Server database.

1 General Information This section discusses how to get started with the EndoTool™ software.

1.1 Registration & Initialization Prior to using the EndoTool™ software, the Administrator is required to initialize the application, which includes completing the following six (6) screens:

If you have already installed and configured EndoTool on another computer, you may export the Unit Administrator Table and Medical Director Table parameters, and then import them on the new computer. This will insure that multiple computers are configured the same.

If the activation code you enter entitles this facility to use a Central Database for storing EndoTool data, this screen allows you to specify the database connection information. If your facility has not purchased the Central Database feature, you will not see this screen.

If you are using a central database, you will be asked to specify the Unit Name of this EndoTool computer. This will allow for synchronization o fusers, parameters, and patients with other computers in the same unit.

If you are not using a SQL Server central database and you have already installed and configured EndoTool on another computer, you may export the Unit Administrator Table and Medical Director Table parameters, and then import them on the new computer. 1.3 Users There are three main users of the system and they are as follows: 1. Level 0 a. Hospital Staff (Nurses): The nurses will be created as users by the administrators and will be the majority of the users of the system. Note: If you are using the Windows Security feature, then users will not be created within EndoTool™. Instead, EndoTool™ will authenticate users against your Active Directory or Windows Domain. 2. Level 1: a. Administrator (Head nurse): This user is responsible for creating the users in the system, printing reports, purging data and the general maintenance of the system. 3. Level 2: a. Medical Director: In addition to the responsibilities of the Administrator, the Medical Director is also responsible for maintaining goals, limits, maximum dosages, insulin types, etc.

1.4 Login As opposed to a traditional application where the user logs in to the system when the application is first launched, the user is presented with the entire application. Once the user selects a task to perform or a table to view he/she will be asked to enter his/her User ID and Password.

2 Menu Bar The menu bar is located at the top of the application and includes three menu options; File, Tools and Help. Each menu option allows different users to perform different tasks.

2.1 File Menu The file menu consists of the following options: 1. Backup 2. Restore Backup 3. Exit

2.1.1 Backup The backup option allows the user to copy the existing data to another location and then use the restore method (see section 2.1.2 Restore Backup) to retrieve it on a new device or computer and continue working. Backups can also be set in the system to be automatic so that the data will always be saved at preset times. In most cases the data will be saved whenever a new patient is added or if patient information is modified.

2.1.1.1 Backing up the System 1. Select Backup from the File menu. a. The backup screen appears. 2. Select the appropriate radio button to indicate where the data should be saved, either to a floppy or a hard disk. a. The default location is “Backup to hard disk” b. The Browse button allows the user to select a location on the hard disk where the data should be saved. 3. Select Cancel to close the window. 4. Select OK to save the data. a. A confirmation message appears “Backup Complete.” 5. Select OK to close the confirmation window.

2.1.2 Restore Backup Restore backup allows a user to retrieve a backup copy of data from a hard drive or a floppy and subsequently work with current data.

2.1.2.1 Restoring the Backup 1. Select Restore Backup from the File menu. a. The restore backup screen appears. 2. Select from the drop down list the folder to Look In. 3. Highlight the appropriate file name. 4. Select Cancel to close the window. 5. Select Open to restore the backup. ©MD Scientific, LLC

a. A confirmation message appears “Restore complete.” 6. Select OK to close the window.

2.1.3 Exit There are three methods of exiting the application; they are as follows: 1. Select the Exit option from File menu. 2. Press the Alt key and the F4 key. 3. Select the X in the upper right hand corner of the screen. The following confirmation appears when the user exits the system:

Select Yes to hide the application in the System Tray and allow alert notifications, No to exit the application or Cancel to continue working with the application.

2.2 Tools Menu The tools menu consists of the following options: 1. Change Password 2. Maintain Users 3. Maintain Parameters 4. Admin Reports

2.2.1 Change Password This option allows the user to change his/her password at any time. In order for the user to change his/her password he/she must first log in to the system. Blank passwords may be entered. If a user chooses to leave his/her password blank, however, the system presents a confirmation screen to confirm that the user wishes to leave the password blank.

1. Select Change Password from the Tools menu. a. The Login screen appears. 2. Enter your User ID and Password to login to the application. a. The Change Password screen appears. 3. Enter a new password. 4. Re-enter the new password. 5. Select the OK button. a. The Change Password screen closes and the new password is saved. i. 2.2.2 Maintain Users This option allows the Administrator to create, modify and delete users in the system. It also provides the Administrator with the option to import or export the user list or print a list of all the users. Note: If you are using the Windows Security feature, then users will not be created within EndoTool™. Instead, EndoTool™ will authenticate users against your Active Directory or Windows network.

2.2.2.1 Adding a New User Administrators are responsible for adding users (mainly nurses) to the system in order that they may access the application. 1. Select Maintain Users from the Tools menu. a. The Login screen appears. 2. Enter your User ID (Administrator) and Password. 3. Select OK to open the maintain users screen. 4. Select Cancel to close the Login screen. 5. Select the User ID field and enter a unique ID (alphanumeric) 6. Select the tab key to move to the password field. 7. Enter a password. a. The password may be left blank but this is not recommended for individual users. 8. Select the tab key to move to the Last name field. ©MD Scientific, LLC

Select the OK button to save the users and close the window. Select the Cancel button to close the window. a. Cancel will only erase the user entered if the Administrator has not completed all the fields. Once all the fields are entered and a new row is selected, the user is automatically saved.

2.2.2.2 Modifying a User If the Administrator entered incorrect information (e.g. spelled a user‟s name wrong), the Administrator can modify the user information from within the Maintain Users screen. 1. 2.2.3.1 Local Computer Table The Local Computer table is the responsibility of the Administrator, which in most practical cases is the head nurse. This data includes all the parameter data that is specific to the local computer, i.e. printers, file locations, etc.

2.2.3.2 Unit Administrator Table The Unit Administrator table is the responsibility of the Administrator, which in most practical cases is the head nurse. This data includes all the pre-specified data that is mentioned in this manual i.e. the number of hours before patient becomes inactive, the number of days to keep patient data before purging, the name of the hospital, etc.

2.2.3.3 Medical Director Table The Medical Director table is the responsibility of the Medical Director. This data includes all the prespecified data that is mentioned in this manual i.e. the goals, limits, insulin types, etc.

Description Select the appropriate radio button either the Audit report or the QA Report to indicate which report should be printed. Enter a date in the fields provided or select the drop down list and a calendar pop-up appears (see screen shot below). The calendar aids the user in selecting the appropriate date. The default value for the start date is one month from yesterday. The default value for the end date is yesterday. Note: The QA report may only be run through yesterday, while the Audit report may be run through the current date. Select OK to generate the report. Select Cancel to close the Admin Reports screen.

2.2.4.1 Audit Reports The unit Administrator runs the Audit report. It can be run at any time. The two main objectives of this report are: 1) Audit nurse activity re. additions and changes to the BS checks for patients 2) verify to the Administrator that nurses are entering BS times relatively close to expected times. Although the system only displays to the nurse the time and date the glucose check was performed, it also stores the expected time and date of the glucose check. 2.2.4.1.1 Producing an Audit Report 1. 2.3.3 Product Updates This option allows Administrators to update the application with a newer version. The Administrator must be connected to the Internet. When the option is selected, the Administrator is led through a series of screens for the update; the software is only updated if a newer version exists on the Internet.

2.3.3.1 Updating EndoTool Software 1. Select Product Updates from the Help Menu. a. The system displays a Check for Updates screen. 2. Select Next to continue or Cancel to close the screen. a. 3 System Tasks The system tasks section is a list of tasks located to the left of the screen below the file menu. It includes the following four system tasks: 1. Add New Patient 2. View Active/Inactive Patients 3. Print Patient reports 4. Perform Housekeeping

3.1 Add New Patient This option allows users to add new patients to the system and record their blood sugar levels and receive insulin dosage instructions. A disclaimer screen is presented to the user before each new patient can be added.

3.1.1.1.4 Yellow Screen – Instructions The yellow screen provides nurses with further instructions, namely the drip, bolus or D50W dosages for the patient, and the expected time of the next glucose check. The instructions screen also displays a message if the patient is considered “stable” by the EndoTool™ application. This message is defined by the unit Administrator in the Unit Administrator table parameters. If a label printer is specified in the Unit Administrator Table, it will automatically be printed at this time. EndoTool currently supports printing a 1” x 2” label. The user can also double click on a glucose check row in the table at any time to review the Calculations Screen for that row.

3.1.2 Modifying Patient Information Any of the Patient information can be modified after it has been saved. The Glucose entries on the Diabetic Record tab, however, cannot be modified or deleted. After a pre-specified amount of time, (the exact value is populated in the Unit Administrator Table) the patient is inactivated. This reduces the number of active patients in the working list. 1. Highlight the patient to be modified in the Active Patient list. a. The Login screen appears. 2. Login to the application. 3. Update the fields on the Patient information tab. 4. Select Cancel to close the screen. a. A confirmation screen appears “Data has changed for the current patient. Do you want to lose the changes?” i. 3.2 View Active / Inactive Patients This option allows Level 0 users (nurses) to toggle between the Active Patients List and the Inactive Patients list. When the application is started, the default value listed in the System Tasks list is View inactive patients because the list located directly below the System Tasks List is the Active Patients List. If the user selects the View inactive patients option the list below the System Tasks changes to the Inactive Patients List and the System Task changes to View active patient.

3.4.2 Purge Old Data This option allows the Administrator to purge all the old data and the old audit reports. Purge data details (i.e. what constitutes old data) are included in the Unit Administrator Table that is maintained by the Administrator. 1. 3.4.3 Inactivate Old Patients The system inactivates old patients who are no longer being checked on a regular basis. When this housekeeping function is performed all the inactive patients will be moved to the Inactive patient list. If the user selects the View Inactive Patients option on the System Tasks then the list will change to the Inactive Patients List. These lists help the nurses to organize their patients and to know who is currently requiring checks and who is not.

5 Alerts This list identifies what action is required of the nurses, namely glucose checks. Once the task has been performed the alert is removed from the list. An alert is listed for any of the following reasons: a. If a Housekeeping report was last run over 24 hours ago. b. If a patient was added without a BS check. c. If a BS check is due within 10 minutes. d. If a BS check is due now. e. If a BS check is past due between 1 and 10 minutes, it is added to the list in orange. f. If a BS check is past due between 11 and 20 minutes, it is added to the list in purple. g. If a BS check is past due between 21 and 120 minutes, it is added to the list in red. If a BS check is past due by more than 2 hours, it is not added to the list.

6 Appendix A – Manual Infusion Calculations 6.1 80-120 mg/dL Goal Range Insulin Infusion Calculations Manual Method Blood Glucose GOAL RANGE 80-120 mg/dL In the unlikely event that the EndoTool™ stops working, the following manual method can be used to calculate the insulin infusion dose for the Goal Range 80 mg/dL to 120 mg/dL. The feedback control features, bolus dose calculations, and frequency determinations will no longer be calculated. In order to calculate the insulin infusion rate obtain the most recent BR & f from the MAR label (last line on label) or from the lower left hand corner of the yellow instruction screen from EndoTool™. With each new blood glucose (BGi) determination and the most recent BR & f, look up the value from the "TABLE BR" and the "TABLE f" based on the BGi reading (left side of table). The dose of insulin is the sum of these two (2) values, one from each TABLE. Keep recording the BG readings, times, and insulin doses in the MAR. The supervising physician may need to alter the BR & f up or down to gain control of this patient's BG, if a prolonged period of time occurs until the local information systems personnel can fix the EndoTool™. Insulin Dose = [Value based on BG from BR Table] + [Value based on BG from f Table] EndoTool™ is a sophisticated insulin dose calculator for patients with an elevated blood glucose who are receiving a relatively constant nutritional support. The primary application is in critical care units where patients have widely varying and changing resistance to the insulin infused to control their blood glucose to a relatively tight, normal range. The primary goal of the software is to calculate insulin doses that will lead to prompt control of blood glucose to a narrow, tightly controlled range as defined by the medical director of the appropriate unit. All insulin doses from EndoTool™ are calculated from one of three standard formulas as follows: {where BGi is the i-th determination of blood glucose (BG) at time I } a. b.

Where "BR" and "f" are patient dependent variables and depend on the sensitivity to insulin and ά (0.03) and ß (0.00008) are constants described in the diabetic literature. EndoTool™ computer calculation is made based on these above formulae only. EndoTool™ Glucose Management System defines expected blood glucose responses to the calculated dose based on the previous blood glucose and pro-rated based on time. Blood glucose responses that are not within the expected response are evaluated based on standard proportional feedback mathematics to alter the "BR" and "f" parameters in limited incremental steps to lead to control of the blood glucose response. With each entry, trend analysis is also used to evaluate for trends in order to address trends that are statistically significant for incremental alteration of the "BR" and "f" parameters.
