(Refer to Para 16 (b) of Joining Instructions) CERTIFICATE TO BE SIGNED BY PARENT/GUARDIANS OF CANDIDATES SELECTED FOR TRAINING AT THE NATIONAL DEFENCE ACADEMY, KHADAKWASLA, PUNE - 411023
I, (Name) father / guardian of (Name)* who is a candidate for training as Cadet in the National Defence Academy, Academy, Khadakwasla for a regular commission in the Army or Navy or Air Force, hereby certify that, I fully, fully, understand that my son/ward@ will do so with my full and free consent and at my own risk, and that tha t I or my son or ward or an y of `my legal heirs shall not be entitled to claim any compensation or other relief from the Gove rnment of India in respect of any injury/infirmity/death, injury/infirmity/death, which my son/ward may sustain in the c ourse of or as a result of training/other activities or where bodily infirmity or death results in the course of or as a result of surgical operation performed upon him or anaesthesia administered to him for the treatment of any injury received as aforesaid or otherwise at the said National Defence Academy, or during any activity undertaken any where in the country or abroad as a cadet of the National Defence Academy. RISK CERTIFICATE: I _________________father/guardian of _________________ who is attending SSB interview hereby certify that I fully understand that my son/ward will, if required, attend Services Selection Board with my full and free consent and at my own risk and that I or my son/ward shall not be entitled to claim any compensation or retest for the same course or any other relief from the Government in respect of any injury which my son/ward may sustain in the course of or as a result of any of the tests given to him at the said Service Selection Board whether due to negligence of any person or otherwise.
